Our office is located in the heart of the Loop, in 200 South Michigan Avenue’s refurbished glass and steel skyscraper. The building is a striking contrast to the ornate turn-of-the-century buildings located along Chicago’s famous Michigan Avenue. Our office has stunning lake views, and is surrounded by commerce and culture, including the world-renowned Art Institute, Orchestra Hall, and Millennium Park.
We are often able to offer discounted parking at the Mellennium Park Garages. We are also located in close proximity to multiple CTA Bus and Train stops.
Unless it is an emergency, I see patients by appointment only. Appointments may be made by calling the appointment desk at (312) 263-5517 the appointment desk will try to arrange your visit at a time most convenient for you. As a courtesy, if you are unable to keep an appointment, please let my office know as soon as possible so that another patient may utilize the appointment time.
You do not have to be referred by another doctor to make an appointment. In fact, many of our patients are sent to us by others we have cared for in the past. However, some prepaid health plans require that you get a referral from your assigned primary care physician to qualify for reimbursement. Please check with your health insurance plan before scheduling an appointment with me to ensure that I can provide your care. Also, please note that not all insurance plans will cover “well women” heath care, in which case, you will be responsible for payment of services rendered to you.
Please notify us 24 hours in advance if you will be unable to keep your scheduled appointment. Failure to do so will result in a $50.00 fee for new and returning patients.
HOW TO CONTACT ME AND STAFF
During normal business hours, I may be at the hospital, in surgery, but our trained support staff will direct your call of message to me or the appropriate personnel.
If your call is an emergency, please state so immediately.
After normal office hours, your call will be forwarded and answered by an answering service. In the event of an emergency your message will be forwarded to me or the physician on call who will call you back as soon as possible. If your condition is not an emergency, you have a non-emergency questions or you need a prescription refill, please call during normal business hours. Our on-call physicians can not attend to non-emergency situations after normal business hours. In addition, we will not refill nonemergent prescriptions of any kind after office hours.
